Summary

Responsible for supporting individuals, families and the sickle cell transition team of professionals to prepare young adults (ages 1718 years) for transition from pediatric to adult medical care with emphasis on patients with complex comorbidities. The Transition Navigator will assess patient needs and collaborate with the multidisciplinary team prior, during, and after transition to ensure a successful transfer of care. The Navigator will assist with patient posttransition needs (ages 1819 years) to ensure continuity of care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

Education/Experience/Licensure

  Education/Formal Training Work Experience Credential/Licensure

REQUIRED:

Associate’s degree in Health, Social Services, Human Services, or related field.

Must have at least one (1) year of experience working in a position requiring interaction with customers or young adults.

Bachelor’s degree.

Prior counseling experience working with youth groups or in community outreach.

Key Job Responsibilities

  • Establishes trusting relationships with patients and their families while providing general support and encouragement.
  • Assesses and addresses the strengths of patients and the barriers to obtaining preventive and follow-up health care.
  • Collaborates with patients to develop and implement a plan to access preventive and follow-up health care.
  • Coordinates with community health care providers, including primary care physicians, other out-patient services, Methodist Le Bonheur Hospitals, St. Jude and ROH Hosptials.
  • Collects, documents, and maintains accurate data related to the program, including performing data entry into clinical databases.
  • Provides regular feedback to team regarding metrics.
  • Maintains regular and predictable attendance, both to required meetings and to assigned clinic visits.
  • Follows up with patients that have multiple missed visits to ensure barriers are addressed.
  • Performs other duties as assigned or requested.

Physical Requirements

  • The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • The physical requirements of this position are: light work - exerting up to 25 lbs. of force occasionally and/or up to 10 lbs. of force frequently.
  • The Associate is required to have close visual acuity to perform an activity, such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
  • The conditions to which the Associate will be subject in this position: The Associate is not substantially exposed to adverse environmental conditions; job functions are typically performed under conditions such as those found in general office or administrative work.
