Summary
This position is responsible for providing psychosocial assessments, interventions, treatment and discharge planning to patients and families. Collaborates and communicates with the healthcare team and involves the patient/family in the plan of care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
