Summary

This position is responsible for providing psychosocial assessments, interventions, treatment and discharge planning to patients and families. Collaborates and communicates with the healthcare team and involves the patient/family in the plan of care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

Education/Experience/Licensure

REQUIRED:	Master’s degree from an accredited School of Social Work.	One year of professional practice (Social Work internships considered practice).	Licensure by the state(s) in which is performed.
