Summary
The RN Patient Care Navigator serves as a consistent care coordinator though out the continuum of care for patients entering or within our health system. Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family. Coordinates communication with providers on behalf of the patient. Meets the needs of the community in relation to education, screening and prevention. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

REQUIRED: |
Graduate of an accredited school of nursing program. |
Strong nursing experience in medical, surgical, oncology, breast health and/or clinical experience. Strong understanding of relevant health care continuums and the various treatment optionS.

Registered Nurse licensed in TN |
PREFERRED: |
BSN preferred |
Outpatient nursing experience preferred.

Specialty certification preferred (Breast Health Navigator)

Knowledge/Skills/Abilities
- Knowledgeable of system’s health resources in order to navigate the patient to appropriate care.
- Must have excellent verbal and written communication skills, a desire to problem solve, and strong conflict resolution abilities.
- Strong computer skills. Scheduling experience a plus.
- Must be a role model in customer service and patient care.
- Displays sensitivity and compassion.

Physical Requirements
- The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
- Must have good balance and coordination.
- The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
- The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
- Frequent invasive and non-invasive patient contact.
- Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
- Ability to react quickly to emergency situations.
