Summary
The BMT Transplant Coordinator serves as a consistent care coordinator throughout the continuum of care for patients entering or within our health system. Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family. Coordinates communication with providers on behalf of the patient. Responsible for coordinating and providing multidisciplinary services to transplant patients and families. Provides overall coordination for the multidisciplinary team involved in transplantation. Meets the needs of the patient in relation to education, screening, prevention and follow-up care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

- Knowledgeable of system’s health resources in order to navigate the patient to appropriate care preferred.
- Must have excellent verbal and written communication skills to convey information to patients, management, physicians, and other associates.
- Strong computer skills. Scheduling experience a plus.
- Excellent problem solving skills
- Must be a role model in customer service and patient care.
- Displays sensitivity and compassion.
- Able to read and interpret complex medical documents.
- Skill and proficiency in applying highly technical principles, science, concepts, and techniques central to the nursing profession.
Key Job Responsibilities
- Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family.
- Coordinates pre & postoperative phases of transplant.
- Provides overall coordination for the multidisciplinary team involved in transplantation.
- Maintains reliable systems to document, track and monitor patient data.
- Performs clinical procedures within the scope of nursing practice and clinical training.
- Responsible for data management activities which will include the collection and submission of data, the oversight of data collection support staff, and the internal audit process to ensure compliance with accreditations.
- Coordinates creation and implementation of the data management plan including continuous process improvement, validation of plans, and completion of necessary documents required for accrediting agencies.
Physical Requirements
- The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
- Must have good balance and coordination.
- The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
- The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
- Frequent invasive and non-invasive patient contact.
- Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
- Ability to react quickly to emergency situations.
- Ability to handle and administer medications, vaccinations and immunizations as needed.
